Duties include but are not limited to the following: Provides direct clinical services in the community to individuals with serious mental illness diagnoses to include symptom management, counseling, psychoeducation, crisis intervention, and support with daily living skills; Works with individuals to support recovery and community integration; Sets and pursues person-centered goals; develops a support system; navigates systems such as Social Security and Medicaid; accesses housing, employment, education, etc.; Collaborates with a multidisciplinary team to support a shared caseload; Participates in daily team meetings to coordinate client care across disciplines; Stays up to date with current and new research and best practices in mental health treatment; Maintains documentation and compliance: provides services in compliance with ACT fidelity standards; completes daily progress notes and treatment plan reviews; Conducts comprehensive assessments to identify mental status, strengths, substance use, functional abilities, and needs to inform service delivery goals; Collaborates with the individuals served, their support network, and the treatment team to develop person-centered treatment plans and sets recovery goals; and Performs other work as required.
