Wellspring is transforming the lives of people with serious mental illness who have been chronically homeless with a team of professionals dedicated to the most effective treatments. First, the ACT team moves people who have experienced chronic homelessness from the streets to affordable housing and then addresses each individual’s complex needs using a best-practice, person-centered approach to restore skills, rebuild lives and maintain housing. The ACT team wraps each individual with intensive services of a medical team, housing and behavioral health. The ideal ACT team candidate will have a belief in recovery from mental illness and an enthusiasm for helping persons with disabilities, rooted in empathy and compassion and demonstrating a careful and caring attitude.

Requirements

  • A personal vehicle is required as the ACT TCM will be responsible for traveling between work sites and transporting clients to regular appointments.
  • KY TCM Training
  • BA or BS in field recognized by DBHDID for TCM: psychology, social work or related field and at least one year post Bachelor's working full-time with adults with serious mental illness OR Master's Degree in related field.

Nice To Haves

  • One year experience working with adults with Substance Use Disorders is preferred.

Responsibilities

  • Assist clients in gaining access to medical, social, educational and other support services.
  • Develops and facilitates the pursuit of individualized recovery goals via assessment, connecting to resources and monitoring progress.
  • Travels throughout the community conducting home visits and connecting clients with resources and services.
  • Collaborates with landlords and other community partners, advocating for client needs and rights.
  • Assists clients in locating, applying for, and moving into housing.
  • Assists clients with illness management, medication management, and independent living skills, providing crisis intervention as needed.
  • Assisting clients with their engagement in physical, mental health, and addiction treatment, ensuring uninterrupted provision of care.
  • Participates and actively engages in weekly / monthly / quarterly staff meetings, on call rotations, and collaborates daily with Wellspring clinical and program staff as well as other providers and sources of support.
