The Senior Case Manager will perform intake assessments, provide outreach, develop individualized service plans, and offer referrals to residents. This role involves assisting residents with benefits applications, budgeting, and advocating on their behalf with Property Management. The position also requires conflict resolution, participation in wellbeing and safety checks, facilitating therapeutic groups, and assisting with program events. Maintaining accurate documentation, assisting with planned exits, and collaborating on new program development are key responsibilities. The Senior Case Manager will also provide back-up coverage for other case managers and share information about new resources.
