About The Position

Performs a wide variety of patient care tasks utilizing a Patient Focused Care model. Follows standardized procedures prescribed in a clinical pathway, including activities related to direct care, diagnostic testing, patient transport and documentation, as directed by RN. Provides direct care to patients under supervision/direction of the registered nurse. Performs a wide variety of tasks using a patient centered model inclusive but not limited to: Assist with Bathing / Personal Hygiene, Assists with ambulation of patient, Assists with feeding, Assist with toileting, Assist with vital signs, Weights, Safety checks, Continuous observation of patients. Exhibits knowledge and skills necessary to provide patient care. Exhibits knowledge of principles of growth and development for patient groups served. Demonstrates knowledge of providing care for specific population / age. Demonstrates knowledge of computer skills. Exhibits ability to provide safe care to individual needs of the patient / family / caregiver in a patient centered environment. Exhibits attributes of caring, empathy and promotion of positive work environment. Embodies a strong work ethic. Exemplifies communication in calm professional manner. Demonstrates strong desire to work as a member of the team. Demonstrates excellence in the vision of customer service in health care. Exhibits good team collaboration skills.

Requirements

  • Must have High School Diploma or equivalent (GED).
  • Must have a valid and active CNA Certification in the State of Connecticut.
  • Must have a valid CPR w/BLS certification from the American Heart Association.
  • Must have a minimum of 1 year of experience working in a long term care and/or behavioral health setting.
  • Knowledge of basic patient care procedures and medical terminology.
  • Ability to work as part of a team.
  • Data entry and documentation skills.
  • Experience working with a psychiatric population.
  • Must be able to participate in PERT.
  • Manual dexterity for phlebotomy and data entry.

Nice To Haves

  • Good communication skills
  • 1 year experience in acute care inpatient environment as Patient Care Assistant or CNA
