About The Position

Asian Health Services (AHS) is seeking a Provider to join our team at a Federally Qualified Health Center (FQHC) as part of our Senior Access and Geriatric Expansion (SAGE) clinic. The ideal candidate will be passionate about providing high-quality care to elderly populations, particularly those facing complex health challenges. You will have extended time during clinic to see your patients and will work within a multidisciplinary team to ensure that our senior patients receive comprehensive, culturally competent care aimed at improving their quality of life.

Requirements

  • Experience in caring for patients with geriatric issues such as cognitive impairment/dementia, falls, polypharmacy, and goals of care
  • Strong communication and interpersonal skills, with the ability to establish rapport and trust with older adults and their families.
  • Commitment to providing high-quality, compassionate care to underserved populations.
  • Valid state medical license and DEA registration in California
  • MD or DO with board certification in internal medicine or family medicine
  • Basic Life Savings (BLS) certification

Nice To Haves

  • Fellowship training in Geriatrics or at least 2+ years of experience taking care of geriatric adults
  • Bilingual in Chinese (Mandarin or Cantonese), Vietnamese, and/or any Asian languages
  • Proficiency with EPIC (EHR).
  • Experience in managing an interdisciplinary team
  • Experience in a community health or federally qualified health center setting is preferred but not required

Responsibilities

  • Provide both primary and consultative direct medical care to geriatric patients, including assessment, diagnosis, treatment, and management of both acute and chronic conditions
  • Lead the SAGE interdisciplinary team, which includes a nurse and community health worker, to provide support for patients and families on addressing unique complex medical and social issues
  • Conduct routine screenings, preventive care, and health assessments to identify issues such as frailty, cognitive decline, polypharmacy, functional limitations, and goals of care.
  • Work collaboratively with outside specialists and other interdisciplinary team members across the organization (nurses, social workers, care coordinators, mental health providers, etc) to deliver person-centered care
  • Advocate for seniors’ health and wellness needs within the community and assist in navigating social services and community resources
  • Maintain accurate and timely documentation of patient care using electronic health record (EHR) systems
  • Adhere to all FQHC regulations, standards, and guidelines to ensure compliance and optimal care delivery
  • Provide mentorship and support to other clinical staff and trainees, fostering an environment of learning and professional growth around geriatric issues.
  • Participate in quality improvement activities and collaborate with the leadership team to enhance patient outcomes
  • Participate in the development and implementation of new geriatric care protocols and best practices in primary care
  • Serve as a geriatric liaison to the AHS Program for All-Inclusive Care for the Elderly (PACE) for transitioning care of patients from FQHC to PACE
