About The Position

Whether it’s finding a job, making a contribution in a volunteer role, establishing a home, or being part of the community, the goal is to assist the person to lead a full and active life. Job Path is seeking an energetic, responsible, and creative person to provide long-term job coaching support to employed individuals with developmental disabilities. This position is a combination of being a part-time SEMP counselor and a part-time Direct Support Professional.

Requirements

BA degree preferred or a combination of related education and experience.
Experience supporting people with developmental disabilities a plus.

Responsibilities

Provide long-term direct support to employed individuals including assisting them in learning new tasks
Support a small caseload of employed individuals including advocacy with employers, benefit assistance, job support and guidance
Encourage Participants to learn new skills to grow at their jobs.
Develop long term working relationship with supervisors and managers.
Work with employers to guarantee their satisfaction with Job Path services.
Visit case load participants at least twice a month
Submit documentation of all services provided for the month for billing purposes in a timely manner.
Be flexible and available to provide support to SEMP participants as needed. At times one might have to work on the weekends.
Work with employer and individual to resolve any problems that may arise during work hours
Write detailed case notes for job coaching services provided for the month
