About The Position
We’re looking for a patient, compassionate, and dependable person to support a young lady who is energetic, crafty, and has an active mind and loves a wide variety of activities. You’ll help provide meaningful activities, encouragement, and support as he works on developing his communication skills. As a Self-Directed Direct Support Professional, you will work one-on-one with individuals to help them achieve the personal goals outlined in their service plan. You’ll play an important role in fostering independence, community inclusion, and person-centered support. Depending on the service plan, you may provide support in the individual’s home, in community settings, or both. This position requires working independently while collaborating with the individual, their broker, and their circle of support. All duties must comply with Agency, Program, and NYS OPWDD guidelines.

Responsibilities
- Provide person-centered support and services to individuals which may include personal care, supervision, home visits, community activities, protective oversight, and related supports.
