About The Position

A Self Directed Services Coordinator drives operational, financial, and regulatory integrity for a 80-person caseload in OPWDD Self-Directed Services—empowering individuals to maximize their independence while maintaining compliance with state and federal healthcare standards. Wildwood Programs is a provider of educational, residential, employment, and other community-based supports to people with disabilities and their families. Requirements

  • Bachelor's Degree in a Human Services-related field preferred. This may be substituted with an Associates’ Degree and one-year of experience in working with people with Intellectual and Developmental Disabilities.
  • Ability to multitask and work independently, while still being part of a collaborative team.
  • Exceptional organizational and prioritization skills
  • Comfort with learning new technology and teaching this technology to others.
  • Knowledge and implementation of Person-Centered Planning skills
  • People skills, and comfort with communicating regularly via all forms of contact.
  • A valid NYS driver's license, daily access to a vehicle and driving record acceptable to the agency insurance carrier's standards are required.

Responsibilities

  • Overseeing a caseload of up to 80 individuals.
  • Processing all reimbursements for individuals, vendors, and staff; and ensuring that reimbursements meet all Medicaid and OPWDD requirements.
  • Reviewing all staff notes on a monthly basis
  • Monitoring expenses and reviewing monthly expenditure reports and sharing monthly reports to each individual’s team.
  • Acting as the main point of contact for the individuals’ self-directed services.
  • Attending biannual Life Plan meetings for all individuals and reviewing Life Plans to ensure all Medicaid and OPWDD requirements for service delivery and billing are met
  • Reviewing Staff Action Plans created by Support Brokers to ensure they match the Life Plan
  • Communicating with various Wildwood departments involved in a person’s Self-Direction services (HR, Finance, Training).
  • Maintaining annual required OPWDD and Wildwood trainings
  • Ensuring all documentation is scanned and saved for a period of 10 years, regular auditing of individual’s files, and maintaining organization of electronic files.
  • Keeping up on OPWDD and Medicaid regulations as they pertain to Self-Direction services, and communicating these regulations to participants as needed.
  • Compiling information for all individuals on their caseload who receive a Housing Subsidy; this may include doing Quality Assurance walkthroughs, home visits, assisting in gathering documents, and completing monthly progress notes for billing of services.
