About The Position

The Screening Coordinator provides screening services at Franciscan Health (Heart Scans, Lung Scans, Vascular Screenings, Cholesterol Screenings, Glucose Screening, Blood Pressure, and Body Fat). The Screening Coordinator provides education and counseling to patients in the Screening Center on health maintenance, disease prevention, early detection, and guides patients to follow-up care within the Franciscan System. The coordinator, also, works as a liaison and builds effective working relationships with patients, healthcare providers, and staff to reduce barriers to patient care, meet patient needs, and improve patient health outcomes. Requirements

  • Bachelor's Degree Health Promotion Required or Bachelor's Degree Exercise Physiology/Exercise Science Required or Bachelor's Degree Public Health Required or Bachelor's Degree Public Health Administration Required or Bachelor's Degree Kinesiology Required or Bachelor's Degree Fitness/Wellness Management Required
  • 1 Year Community Health Required
  • 1 Year Clinical Education Required
  • Basic Life Support Program (BLS) - American Heart Association Required within 90 days

Nice To Haves

  • Master's Degree Public Health Preferred
  • High School Diploma/GED Hired on or after 12/4/2022 Bachelors Required
  • 1 Year Biometric Screening Preferred or Hired on or after 12/4/22 Bachelors required

Responsibilities

  • Educates patients on screening results, preventative health maintenance, smoking cessation and NRT.
  • Reviews all radiology reports for incidental findings and makes appropriate referrals.
  • Documents accurate documentation of each patient encounter.
  • Ensures all patient records are accurate, up to date and meet all regulatory and confidentiality guidelines.
  • Performs patient outreach regarding required follow-up testing and screening compliance.
  • Schedules and coordinates patient exams and follows up with healthcare professionals and patients regarding the results.
  • Answers questions as appropriate and serves as liaison to the physician for additional questions and concerns.
  • Communicates all findings to the patient’s primary care physician.
  • Monitors screening guidelines and compliance according to the Franciscan system and national guidelines.
  • Inspects the area for compliance with policies, procedures, regulations, and standards and takes corrective action when violations occur.
  • Ensures accurate data submission into appropriate database systems.
  • Suggests policies or practices that should be implemented to support increased quality of care; utilizes evidenced-based protocols in developing policy and procedures.
  • Collaborates with FPN and Hospital administration to ensure physician buy-in and seamless patient continuum of care.
  • Collaborates with vascular/cardiac/thoracic surgeons, cardiologists, oncologists, pulmonologists, radiologists, hospital administration and physician offices to identify growth opportunities for the screening program.
  • May be required to maintain screening supplies inventory by checking stock and outdates to determine inventory level, anticipating needed supplies and placing orders for required inventory.
