The School Specialist will be responsible for coordinating and preparing educational progress reports for patients' schools, collaborating with an intradisciplinary team on transition plans back to school, and consulting with patients and families regarding re-entry plans. This role also facilitates the re-entry process following discharge and participates in patient care conferences. The specialist will meet new patients, gather pertinent information through observation and interview, assess educational needs in the classroom and at bedside, and make appropriate recommendations. Additionally, they will make internal and external referrals as indicated, assess educational level and cognitive status of patients, and identify physical and cognitive readiness for participation in school programs. The role involves formulating appropriate educational goals, developing curriculum based on individual patient needs, and implementing educational programming to maximize growth and development that is age/grade level appropriate.
