Provide school social work services to students who are at-risk for school failure and/or are demonstrating social/emotional/behavioral problems within the school setting. Provide direct and indirect school social work services as a related service in support of special education. Assists school staff in developing and implementing interventions to allow students with disabilities to be successful in an educational setting. Conducts social/developmental assessments of students referred for special education, assists in determining eligibility for special education. Communicates with staff, administration and parents concerning students’ social, emotional and behavioral progress. Provides information to students and parents about services available within the community. Makes referrals to community providers as appropriate. Maintains an accurate record of student, parent and other contacts. Conducts students risk assessments. Coordinates home, school and community resources in addressing the crisis situation. Act as liaison between the schools, physicians and public health entities. Coordinate mental health documents and mutual health related communications between hospitals, medical agencies, and the school. Attend all meetings and professional development as directed by the Director of Special Education. Conduct himself/herself in a professional manner commensurate with his/her position and project a good public image of the School District in dealing with the community and employee groups. Assume other responsibilities as determined by administration and as allowable by funding sources.
