About The Position

Provides skilled nursing care to the individuals at school day program. Acts as a liaison between Nursing Department and day program staff/management, while overseeing all health related day programing medical concerns.

Requirements

  • Current licensure as RN/LPN in PA
  • Current American Heart Association issued Basic Life Support or Advanced Cardiovascular Life Support certification.
  • Current Driver’s License
  • Must be able to transfer a minimum of 50 pounds

Responsibilities

  • Provides skilled nursing care to individuals in the day program, including but not limited to assessment of acute and chronic concerns and medication and treatment administration.
  • Provides in-services on health issues to day program personnel.
  • Completes and maintains documentation on individual care services and keeps confidential medical information secure.
  • Communicates to Nursing Dept., appropriate day program personnel including teachers and administration of any acute and emergent health issues that arise with individuals enrolled at the day program.
  • Communicates all emergency and health related issues by the end of their working shift with residential Nurse.
  • Contacts the guardians of individuals enrolled at the day program if any acute or emergent health issues arise.
  • Demonstrates computer proficiency.
  • Attends meetings for individuals as requested that occur in day programs, coordinating input with the professional disciplines at Woods.
  • Collaborates with guardians, Medical Providers, and multidisciplinary professional staff to implement individualized care for those enrolled at the day program.
  • Collaborates with Medication Trained Staff that are assigned to the day programs, orienting and educating them on specific medical treatments, changes in medications, new medications, etc.
  • Maintains a safe and clean working environment, managing organization of office, medical records, and nursing supply inventory. Anticipates medical supplies needed and orders as necessary.
  • Ensures operation of medical equipment by verifying emergency equipment availability, troubleshooting malfunctions, requesting repairs, and maintaining equipment inventories on a routine basis.
  • Participates in any regulatory inspections with preparation and monitoring of health related documentation.
  • Performs all other duties as assigned.
