About The Position

NYAP is seeking a full-time Therapist for its STAR School Based Team in Cleveland, Ohio, with an opportunity at Imagine Bella. Under the general supervision of the Clinical Supervisor or designee, the therapist works with youth and families to support safe and successful community living. The role involves counseling and therapy, advocacy, and support, in coordination with youth, families, foster parents, and community partners. The therapist provides community-based services and coordinates care across mental health, medical, educational, psychological, vocational, and social service needs. The position requires timely documentation, meeting productivity thresholds, maintaining relationships with foster and birth families, and contributing to family reunification or stabilization. It also includes safety assessment, crisis response, behavioral stabilization, quality-improvement activities, and weekly supervision.

Requirements

Master’s degree in Social Work or a comparable Human Services field from an accredited institution.
Valid advanced professional license as required and accepted by state or contract regulations.
Valid driver’s license, reliable personal transportation, automotive insurance, and a good driving record.
Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability.
Willingness to work flexible and non-traditional hours in service of foster caregivers, families of origin, and persons served.
Ability to be flexible and responsive to youth, foster caregivers, system partners, and internal customers.
Ability to remain objective and professional in all areas of job function.
Ability to work effectively and respectfully with people from diverse cultural, socioeconomic, ethnic, religious, and racial backgrounds.
Proficiency with desktop and laptop computers, smartphones, tablets, printers, fax machines, photocopiers, and word processing, spreadsheet, and database software.

Nice To Haves

Two years of direct-service work experience with youth and families.
Fluency in English and Spanish.

Responsibilities

Provide individual, group, and family mental health services, case management, transportation, and other professional services while meeting or exceeding established outcome thresholds.
Travel as required to provide community-based services to and on behalf of youth and families in compliance with organizational, contract, and regulatory requirements.
Complete required documentation in a timely manner.
Consistently achieve established productivity thresholds.
Coordinate and monitor community services for youth and families, including mental health, medical, educational, psychological, vocational, and social services.
Regularly review and modify youth-based plans to support progress toward written goals.
Establish and maintain strong relationships with assigned foster parents and families of origin.
Assist with family reunification or stabilization efforts consistent with the youth’s case plan.
Provide safety assessment, crisis response, and behavioral stabilization services as required.
Participate in Continuous Quality Improvement activities monthly or quarterly.
Actively participate in weekly supervision.
