About The Position

This position is for a School Based Social Worker (LISW) or Clinical Counselor (LPCC) at Mason Middle School. The role involves providing comprehensive psychosocial services to students and their families within a school setting. The social worker or counselor will collaborate with the healthcare team, families, and community resources to support patient well-being and treatment adherence. Key responsibilities include assessments, interventions, interdisciplinary collaboration, and resource support.

Requirements

  • Master's degree in Clinical Counseling or Social Work
  • 2+ years of work experience in a related job discipline
  • LPCC or LISW License in the state of Ohio
  • May require clinical license, certification or training

Responsibilities

  • Performs psychosocial assessments considering family, social, and medical history, and maintains current knowledge of medical and mental health diagnoses.
  • Reports suspicions of abuse and neglect to legally mandated authorities.
  • Completes documentation with accuracy and clarity.
  • Collaborates with the health care team in developing treatment plans and identifying appropriate resources.
  • Maintains fluent knowledge of internal and external resources and provides accurate information to patients/families/caregivers.
  • Provides condition-specific and related medical, financial, educational, and social supportive resource information.
  • Evaluates the effectiveness of clinical interventions and makes necessary changes.
  • Provides psychosocial services as identified in the patient's comprehensive plan of care.
  • Offers supportive counseling to improve emotional coping and adherence to the treatment plan.
  • Educates patients/families/caregivers and healthcare team members about treatment options.
  • Provides self-management support to high-risk/complex patients/families.
  • Provides groups and programs for special populations as needed.
  • Offers anticipatory guidance and planning to reduce psychosocial barriers to discharge.
  • Coordinates and facilitates family-centered care team meetings.
  • Provides crisis assessment and interventions to restore equilibrium.
  • Collaborates with referral sources and health care team members.
  • Works towards system improvement and promotes patient/family/caregiver well-being.
  • Identifies and advocates for the needs of patients/families/caregivers within the healthcare system and community.
  • Acts as a liaison between patients/families/caregivers, the medical team, and outside agencies.
  • Mediates within the medical team on behalf of the patient/family/caregiver as needed.
  • Empowers patients/families/caregivers to problem-solve by exploring options of care and alternative plans.
  • Facilitates communication and coordination among healthcare team members, involving the patient/family/caregiver in decision-making.
  • Partners with the patient, family, and healthcare team in the safe transition of care.
  • Educates healthcare professionals and students on the psychosocial aspects of patient care and the role of social work.
  • Promotes a positive work environment.
  • Participates in professional growth opportunities, attending meetings, conferences, and in-service programs.
