About The Position

Provide individual and/or family mental health assessment and treatment for children, adolescents, and their families in the school-based health center. May provide services to child and adult clients as requested at main sites.

Requirements

  • Graduate degree in Social Work, Counseling or Human Services.
  • CT License required.
  • CPR/BLS Certified
  • Excellent organizational and communication skills.
  • Computer literate including Microsoft Office; ability to document in an EHR.
  • Ability to work well with culturally diverse populations.
  • Ability to utilize Electronic Health Record (EHR)

Nice To Haves

  • LCSW, LPC, LMFT preferred.
  • Experience in providing services to medically underserved and culturally diverse populations.

Responsibilities

  • Provides psychosocial assessment, treatment planning, therapy, and appropriate referrals to youth ages 5 through 18 (or upon completion of high school).
  • Facilitates group sessions (e.g. CBITS/Bounce Back) as assigned.
  • Refers clients for psychiatric care, Southwest services, and outside providers/services as necessary.
  • Consults with supervisor, psychiatric providers, department members, and medical staff regarding client care.
  • Participates in behavioral health child rounds, individual supervision, and department/agency meetings.
  • Provides advocacy for clients within the school system (e.g. 504 and IEP meetings as requested by the client’s guardian) and other systems such as DCF.
  • Maintains client records in the EHR in accordance with SWCHC timeframes/policies and provides other documentation as requested.
  • Performs other duties as assigned.
