About The Position

As a School Based Behavior Health Coach, you will empower, uplift, and help young people overcome challenges stemming from mental illness, trauma, or addiction. You will provide additional guidance and support as they navigate multiple systems, particularly the school setting. The BHC will work closely with the therapeutic team, family, and school to support the learning and applying of skills outside of the clinical setting. This position is responsible for providing targeted support services, including case management, rehabilitation, and wellness services, within a child-centered system of care. The goal is to assist in seamless engagement into outpatient services and aftercare to prevent further out-of-home or out-of-school placement. The School Based Behavioral Health Coach may work with the identified consumer and family in their homes, schools, and communities, incorporating the family and other social supports in treatment and focusing on strengths, consumer preferences, and culture. This position is supervised by the respective Director of School Based Services.

Requirements

Bachelor or Master Degree in Education, Psychology, Social Work, Sociology, Criminal Justice or similar field approved by ODMHSAS; or A current license as a registered nurse in Oklahoma; or A Bachelor's or Master's degree in any field and proof of active progression toward obtaining a clinical licensure Master's or Doctoral degree from a regionally accredited college or university recognized by the USDE.
One year of work experience with juveniles in the mental health or social service field preferred
Behavioral Health Case Management Certification and Behavioral Health Wellness Coach Certification is required within the first three months of employment and must be kept current annually.
Current certification as a Behavioral Health Case Manager II as described in ODMHSAS Standards ad Criteria for Certified Behavioral Health Case Managers 450:50-3-1; and Current certification as a Behavioral Health Wellness Coach as described in ODMHSAS Standards ad Criteria for Certified Behavioral Health Case Managers 450:50-5-145.
An expectation of core competencies in relation to children and families, as well as individuals with co-occurring disorders, trauma informed care and collaborative care.
Knowledge of the principles and practices of social work and/or psychology; and of current social and economic problems, principles of family economics and budgeting, human relations, community, state and national health and welfare resources, social assistance requirements, and psychological terminology.
Knowledge of transitional and/or aftercare needed after a crisis or hospitalization and the unique situations that may arise as consumers attempt to engage in outpatient services.
Ability to evaluate situations and adopt an effective course of action, to delegate responsibilities, organize and present facts effectively.
Knowledge of the elements of personal wellness and how to collaborate with others to identify their personal strengths and goals within the Eight Dimensions of Wellness.
Able to establish working relationships within an integrated team to coordinate and facilitate care by assisting with assessing consumer needs, evaluating treatment options, creating care plans, coordinating care with internal and external providers, and monitoring progress.
Ability to work effectively and courteously with consumers and families from various cultures and backgrounds, and to exercise good clinical judgment.
Specialized training and/or certification in mental health services or related field may be required in order to provide adjunctive services, ie. Behavioral Health Aide, Systems of Care, Wraparound training.

Responsibilities

Coordinate with team to develop and provide a behavioral management plan that includes communication, self-help, safety, substance use decisions, and daily living skills based on the individual needs of the child.
Provide varying types of individualized case management, rehabilitative and wellness services in various settings to consumers, families and groups to promote emotional or psychological wellbeing.
Assists in scheduling, coordinating, and participating in family and school team meetings as needed and ensure formal and informal supports are included and part of the team.
Monitors and evaluates the needs and abilities of the consumer and assists them to function more adequately through use of their own strengths and through use of community resources.
Will recognize the individual and family have different barriers to social, economic, and educational opportunities and has varied levels of need to overcome those barriers.
Assists consumers to adjust and adapt themselves to their environment, care, and treatment.
Identifies barriers to treatment and assists consumer in problem solving and engaging in outpatient care to increase academic performance, reduce office referrals/suspensions, increase pro-social behavior, and increase overall mental and physical wellness.
Completes all agency documentation of services in a thorough, accurate and timely manner.
Serves as a role model for wellness behaviors.
Applies the principles and processes of coaching when collaborating with others.
Collaborate with team to provide referrals to community resources for housing, employment, medical, nutrition education, weight management, Oklahoma Tobacco Helpline, and other community or wellness-related services and resources.
Develop cooperative working relationships within the local community and school to enhance consumer and family access to resources that support the individualized Care Plan goals.
Employee must be willing and able to perform all job-related travel normally associated with this position.
Must meet agency standards of productivity and fiscal responsibility.
Prepares for and actively participates in huddles, Care Planning, and staff meetings.
Must meet “satisfactory” expectations at any scheduled job performance evaluation, coaching and/or Performance Improvement Plans.
Assessment and appraisal of quality of work will be evaluated through training, skill, chart review or special purpose.
Must complete Lighthouse required training and resulting follow up and consultation as required at hire, annually and as directed by Supervisor.
Performs other reasonably related duties as assigned by the immediate supervisor or other management as requested.
Must perform the specific job duties as listed above to meet position expectations.
In addition, must possess skill in working cooperatively and effectively with consumers, staff, management and other professional and community groups; in exercising mature judgment in dealing with people; in presenting ideas clearly and accurately; in reading and comprehending the English language; and in communicating effectively, both orally and in writing.
