The position is based at a high school campus, with occasional work at Waco Family Medicine sites, and reports to the Project Manager, School-Based Services. The clinician also tracks caseloads, service utilization, referral closures, and outcomes for program dashboards and supports grant and funding requirements as applicable. Example measures of success include a time-to-first-visit of no more than 10 school days; at least a five-point improvement on PHQ-A or GAD-7 for students with elevated baseline scores; positive school staff feedback on collaboration and communication; and adherence to FERPA/HIPAA data-sharing agreements.
