About The Position

The School Nurse provides clinical services at School Based Health Clinics during school hours and transitions to the designated Community Health Center after school and on non-school days. The Pediatric Licensed Practical Nurse or Registered Nurse is responsible for ensuring the delivery of quality healthcare services at the assigned clinic site(s), including School Based Health Clinics, under the direction of the Site Manager (SM), Director of Nursing (DON), and Pediatric Provider. The role participates in the patient’s care plan by utilizing the nursing process to carry out interventions to improve patient health outcomes. The nurse must follow and adhere to all federal, state, and local regulations, and comply with the standard of care, adhering to InclusivCare’s guidelines, protocols, policies, and procedures, including those of certifying and accrediting agencies and bodies. The nurse also acts as a liaison between clinical and non-clinical staff members, providers, patients, hospitals, and other external health care agencies.

Requirements

  • Experienced in Pediatrics, Medical/Surgical Nursing, Emergency Room, Ambulatory Care or School Nursing.
  • 3 to 5 years of experience required.
  • Licensed Practical Nurse or Registered Nurse (preferred) with successful completion of an approved and accredited Nursing program.

  • Must be an LPN or RN (preferred) with a current Louisiana State Board of Nursing license.
  • Basic Life Support (BLS) Certification required.

Responsibilities

  • Assist providers with clinic visits, including obtaining accurate vital signs, focused precise intakes, and assessments.
  • Conduct Point of Care Testing (POCT).
  • Conduct daily Quality Controls (QC), temperature checks, and logs.
  • Administer medication utilizing the 10 medication rights to reduce the potential for errors.
  • Set up and assist with ordered procedures.
  • Conduct patient Care Management.
  • Manage and adhere to VFC and LINKS program standards and procedures.
  • Document with meticulous attention to detail in the Electronic Medical Record (EMR).
  • Notify the provider immediately of observed changes in the patient’s medical condition.
  • Provide age-specific, quality, patient-centered care with sensitivity and respect for the diversity of human experience, incorporating the patient and family’s unique differences into their individualized direct plan of care.
  • Promote and model Customer Service Excellence.
  • Represent the organization in a positive and professional manner at all times.
  • Accommodate unforeseen patient/clinical activity in the clinic’s fast-paced environment.
  • Communicate pertinent information and notify the SM and DON of clinic issues that may arise immediately.
  • Facilitate the clinic’s morning meetings and afternoon huddles.
  • Interact with medical and non-clinical staff members to deliver quality patient care.
  • Maintain current clinical knowledge, demonstrating competency and functioning as a clinical resource.
  • Seek professional growth by participating in Continuing Education (CE) activities annually.
  • Practice and maintain all federal, state, and local regulations, guidelines, protocols, and standards of care ensuring compliance and continuity of patient care.
  • Consistently comply with the organization’s policies and procedures.
  • Be responsible for the clinic’s ongoing Environment of Care, Infection Control, Quality and Process Improvement processes.
  • Prepare and complete necessary reports and documents thoroughly, accurately, and in a timely manner.
  • Participate in committees and task forces; and attend professional meetings and training, as required.
  • Collaborate with the Director of Nursing and Pediatric Provider to develop and implement Clinic Goals, Operational Policies and Procedures, and Organizational and Community Specific Projects.
  • Support current organizational initiatives, such as Emergency Preparedness, exhibiting creativity and flexibility in times of change.
  • Travel to any InclusivCare location as assigned and/or needed.
  • Participate in community outreach activities as needed.
  • Perform other duties and tasks as assigned and/or needed.
