Location This position is based in-office in Crestview, FL, supporting Covenant Home Health Care patients through our partnership with Concierge Home Care. Position Overview: LPN / Scheduling Coordinator This is a full-time, in-office position, Monday–Friday, 8:30 AM to 5:00 PM, with flexibility to assist in the field and complete LPN patient visits as needed. As an LPN / Scheduling Coordinator, you will play a critical dual role—supporting daily scheduling and operational coordination while maintaining your clinical skills by providing direct patient care when needed. This position is ideal for an LPN who enjoys organization, teamwork, and balancing administrative responsibilities with hands-on nursing care.

Requirements

  • Unencumbered Florida Licensed Practical Nurse (LPN) license (required)
  • Valid driver’s license, reliable transportation, and active auto insurance (required)
  • Strong organizational, communication, and time-management skills

Nice To Haves

  • Home health experience preferred, but not required

Responsibilities

  • Coordinate and manage clinician schedules to ensure optimal patient coverage
  • Communicate with patients, caregivers, and clinical staff to confirm visits and address scheduling changes
  • Collaborate closely with Covenant leadership and Concierge clinical teams to support daily operations
  • Assist with workflow coordination to ensure continuity and quality of care
  • Provide hands-on nursing care to patients in their homes under the direction of a Registered Nurse
  • Follow individualized, goal-driven plans of care established by the RN
  • Deliver skilled nursing services, including: Acute and chronic disease management Medication administration and management Post-surgical support Pain management Wound care
  • Complete timely and accurate documentation using streamlined tools and dictation services
