The Schedule Coordinator schedules patients with field clinicians and coordinates communication among field staff, patients, physicians, referral sources, caregivers, and other service providers to support proper care coordination and continuity of care. The role also manages the on-call notebook and hospitalization logs to improve communication among stakeholders. The position requires a high school diploma or equivalent, an active LPN or LVN license, and relevant clinical or home-care experience. Employees must maintain required certifications, a valid driver license, automobile liability insurance, and dependable transportation. The role requires communication and technology skills, including use of mobile devices and Microsoft Office. In-office attendance is essential except for designated full-time remote positions; occasional remote work for temporary needs or crises does not waive this requirement.
