Requirements

  • High School Diploma or equivalent.
  • Licensed practical or vocational nurse.
  • Valid clinical license, per state requirements to complete this job in accordance with applicable board’s rules.
  • Minimum of one year of clinical experience in a healthcare setting.
  • CPR certification, as required by position.
  • Minimum of one year of home health, hospice, or pediatric experience within the last 24 months with demonstrated experience and understanding of staffing and scheduling requirements related to home care services.
  • Valid state driver license in the current state of residence.
  • Automobile liability insurance, as required by law.
  • Basic demonstrated technology skills, including operation of a mobile device.
  • Dependable transportation in good working condition and be able to safely drive in all types of weather conditions.
  • Oral communication, written communication, fluency in English, active listening.
  • Demonstrated intermediate technology skills. Especially Microsoft office, including word and excel.

Nice To Haves

  • Previous experience using Homecare Homebase (HCHB).
  • Bilingual (English/Spanish) with the ability to facilitate clear communication with patients and caregivers, improve service delivery, and support positive patient outcomes.

Responsibilities

  • Responsible for scheduling patients to field clinicians.
  • Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of care.
  • Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
