About The Position

The Ryan White Housing Case Manager is responsible for providing comprehensive housing-focused case management services to individuals living with HIV/AIDS under the Ryan White and HOPWA programs. This role ensures clients obtain and maintain stable housing by assessing needs, developing individualized service plans, coordinating resources, and providing ongoing support. The Case Manager works collaboratively with internal teams and community partners to improve client health outcomes, housing stability, and overall quality of life.

Requirements

  • Expertise in using the Microsoft Office software suite is required.
  • Must possess a strong understanding of HIPAA regulations.

  • Bachelor’s degree in Social Work, Human Services, Public Health, or related field required (or equivalent experience).
  • Minimum of 2 years of experience in case management, housing services, or working with vulnerable populations.

Nice To Haves

  • Experience working with individuals living with HIV/AIDS preferred.
  • Knowledge of Ryan White and HOPWA programs strongly preferred.

Responsibilities

  • Maintains housing eligibility requirements for clients.
  • Conduct comprehensive intake assessments to determine housing needs, eligibility, and barriers.
  • Develop individualized service and housing stability plans in collaboration with clients.
  • Provide ongoing case management, including regular follow-ups and reassessments.
  • Maintain accurate, timely, and detailed case documentation in electronic health records.
  • Monitor client progress and adjust plans as needed to support long-term housing stability.
  • Conducts home visits.
  • Assist clients with applications for housing assistance programs, including Tenant-Based Rental Assistance (TBRA), Permanent Housing Placement (PHP), Short-Term Rent, Mortgage, and Utility Assistance (STRMU).
  • Provide education on lease agreements, tenant responsibilities, and coordinate housing stability strategies.
  • Mediate and resolve conflicts between clients and landlords when necessary.
  • Collaborate with medical providers, case managers, and multidisciplinary teams to ensure coordinated care.
  • Participate in case conferences, team meetings, and care coordination activities.
  • Stay current on Ryan White, HOPWA, and HUD regulations and best practices.
  • Ensure compliance with Ryan White, HOPWA, and organizational policies and procedures.
  • Maintain client confidentiality in accordance with HIPAA regulations.
  • Participate in trainings, community meetings, and quality improvement initiatives.
