About The Position

NorthKey Community Care is seeking a Rural Health Transformation Project Director to coordinate and oversee the Rural Health Transformation project grant. This role involves developing relationships with various health entities, coordinating training, streamlining crisis response pathways, and ensuring timely reporting and data analysis. The position also requires maintaining strong partnerships with community stakeholders and promoting alignment across grant-funded initiatives. The director will adhere to NorthKey policies and procedures, maintain confidentiality, and represent the organization professionally.

Responsibilities

  • Coordinate and oversee the Rural Health Transformation project grant.
  • Develop relationships with Health Departments, FQHCs, EMS, hospitals, and related systems focused on managing crisis response.
  • Coordinate with the Training and Education Coordinator and community partners to ensure trainings are offered at regular intervals to community partners.
  • Ensure ongoing access to trainers for Code Red, Mental Health First Aid, Youth Mental Health First Aid, and Trauma-Informed Care.
  • Streamline crisis response pathways in hospital and EMS systems across NorthKey’s five rural counties: Carroll, Owen, Pendleton, Grant, Gallatin.
  • Develop care coordination pathways across systems for individuals experiencing a crisis.
  • Create procedural workflows that can be utilized by area hospitals and EMS to increase warm handoffs and referrals during episodes of care.
  • Ensure timely submission of all required programmatic, performance, and financial reports, and coordinate responses to monitoring findings.
  • Collaborate with the Program Evaluator to develop appropriate data tracking systems and manage review of data for the grant.
  • Develop and maintain strong partnerships with community stakeholders, including public health agencies, crisis services, hospitals, law enforcement, schools, and advocacy organizations.
  • Promote alignment and coordination across grant-funded initiatives to maximize organizational learning, efficiency, and sustainability of suicide prevention efforts.
  • Ensure complete, accurate, and timely submission of all reports.
  • Oversee project data collection, reporting, and analysis to ensure compliance with grant requirements and to inform continuous quality improvement and project implementation.
  • Coordinate with BI Analysts and Mission Driven Data representatives to create necessary data reporting systems.
  • Other duties as assigned.

  • Maximize efficiency and effective use of time in maintaining accurate and punctual time reporting.
  • Attend internal meetings relating to responsibilities and duties.
  • Maintain a professional and business-like manner in dealing with all internal staff, Board members, state and governmental staff, sub-contractors and vendors of NorthKey.
  • Represent NorthKey in a professionally appropriate manner.
