The CDC Foundation is seeking a Rural Health Transformation (RHT) Content Coordinator to support the Kentucky Rural Health Transformation Program, a multi-year statewide initiative to modernize rural health systems and improve access, quality, and sustainability for rural Kentuckians. The program includes five initiatives: Rural Community Hubs, PoWER (perinatal health), Rapid Response to Recovery (behavioral health and crisis care), Rooted In Health (integrated/oral health), and Crisis To Care (community trauma care and EMS), coordinated by a central program office. The role combines hands-on content production with participation in communications strategies and execution plans, supporting clear, consistent, coordinated messaging across the program. In partnership with the Communications Lead, the Coordinator is also the day-to-day coordination lead for the Engagement and Outreach Coordinator team, helping field-facing staff obtain materials, messaging direction, and support for representing the program in Kentucky communities. The position is currently remote, with Kentucky residents only and some travel required. The role requires operating a motor vehicle, a valid driver’s license, acceptable insurance, and compliance with the CDC Foundation’s motor vehicle record review and driver safety policy. The position end date is June 30, 2028. The project is funded by CMS/HHS, and responsibilities may evolve as program needs change, subject to written approval by the CDC Foundation and Kentucky Department for Public Health.
