About The Position

The Rotating Pharmacist is responsible for providing quality pharmaceutical care/services to patients and health care professionals at any ARH or affiliated unit as assigned.

Requirements

Bachelor of Science degree in Pharmacy, PharmD, or other advanced degree required.
Licensure eligibility in states where expected to practice pharmacy required.
Responsible for properly assessing patients' pharmaceutical needs and providing pharmaceutical care.
Responsible for the accurate and proper compounding and dispensing of prescribed medications and solutions.
Responsible for the proper inventory and record keeping of medications.
Responsible for properly counseling patients, patients' families, and appropriate healthcare professionals regarding drug therapy.
Strong knowledge of medication management, drug interactions, contraindications, and therapeutic monitoring.
Ability to accurately review, verify, and evaluate medication orders.
Strong communication and patient education skills.
Ability to collaborate effectively with physicians, nurses, and other healthcare professionals.
Knowledge of state and federal pharmacy regulations and standards.

Responsibilities

Supervises and performs the functions involved in the inpatient drug distribution program.
Supervises and performs the functions involved in the accurate and efficient dispensing of outpatient prescriptions.
Supervises and performs the functions involved in the I.V. Admixture Program, including the accurate and efficient preparation and delivery of total parenteral nutrition solutions, chemotherapy, antibiotics, and other medications for intravenous and irrigation therapy.
Reviews and verifies medication orders for accuracy and completeness.
Monitors patient drug therapy to detect and prevent drug interactions, contraindications, and other adverse reactions.
Makes appropriate interventions to improve patient care.
Uses and demonstrates proficiency in the operation of the pharmacy computer system.
Compounds and dispenses medications and other pharmaceutical agents/products.
Obtains pertinent patient medication and medical information.
Provides current and accurate drug information to patients and healthcare professionals.
Maintains appropriate records and inventory on all drugs and pharmaceutical agents.
Monitors the proper storage and distribution of patient floor stock, controlled substances, and emergency medications.
Communicates effectively with patients and healthcare professionals.
Trains and supervises pharmacy support personnel as assigned.
Serves on hospital committees as assigned.
Assumes the duties of the Director of Pharmacy when assigned.
Provides pharmaceutical care in assigned locations as directed by Central Pharmaceutical Services.
Performs other related duties as assigned.
