About The Position

This role is responsible for various aspects of patient care, including electronic medical record management, daily nursing schedules, patient triage, care coordination, and case management. The position involves analyzing preventative screening data, providing guidance to clinical teams, and participating in quality improvement initiatives. The RN will also support specific programs like the Coleman initiative and Patient Centered Medical Home standards, and perform other duties as assigned by management.

Requirements

  • Graduate of an accredited RN nursing program.
  • Current New York State license in Registered Nursing.
  • Completion of additional training for care management, SBIRT, Depression and Anxiety screening.
  • Willingness to accept direction from providers and Sr. VP/ CNQO.
  • Ability to work well and with compassion in stressful situations.
  • Maintain a professional and courteous demeanor with both patients and co-workers.
  • Keen awareness of the importance of confidentiality in all aspects of the position.

Responsibilities

  • Manage all aspects of the electronic medical record, including training, compliance monitoring, and timely follow-up.
  • Oversee the daily nurse visit schedule, ensuring timely billing and charting.
  • Perform telephone and on-site triage, addressing patient questions for behavioral health and medical patients.
  • Support the monitoring of patients awaiting behavioral health appointments, as identified for follow-up by the PCP.
  • Coordinate care management for identified patient populations, including both medical and behavioral health patients.
  • Provide case management for patients identified through the care team, developing coordinated plans of care to improve health outcomes.
  • Analyze preventative screening data for designated patients and provide guidance to clinical support and providers.
  • Provide feedback to the nurse manager regarding team members' patient-centered medical home activities.
  • Participate in quality improvement, in-service workshops, continuing education, and peer review.
  • Assist with and promote processes associated with the Coleman initiative.
  • Assist with and promote processes associated with Patient Centered Medical Home standards.
  • Perform any other duties and responsibilities as requested by management and supervision.
