Requirements
- Current RN License
- 2 years of experience in home health or related field
- Experience working with special needs population
- Knowledge of current home management and personal care methods
- Must be able to pass a Criminal Background Check
- Tuberculosis Screening Test Results
- Physical
- CPR/First-Aid
- Recent 7 year Motor Vehicle Report copy if hired
Responsibilities
- Ensure the individual care plans are implemented
- Conduct a face-to-face evaluation of individuals as needed
- Make weekly home visits and monthly physical assessments
- Follow-up with appointments, prescriptions, physician orders, discharge orders, etc.
- Document service provided to each individual in a timely manner
- Other related duties as assigned
