About The Position

The primary responsibility of this RN position is to supervise the day-to-day nursing activities of the facility during the assigned tour of duty. This supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern the facility and as required by the Director of Nursing (DON) and/or Assistant Director of Nursing (ADON) to ensure the highest degree of quality care is maintained at all times. The role involves administrative, committee, personnel, nursing care, staff development, safety and sanitation, equipment and supply, care plan and assessment, budget and planning functions, as well as supervisory requirements. Requirements

Nursing Degree from an accredited college or university.
Training in rehabilitative and restorative nursing practices.
2 years or more experience, preferably in a long-term care facility.
Ability to read, write, speak, and understand the English language.
Ability to read technical procedures.
Ability to read and comprehend policy and procedure manuals.
Ability to effectively present information and respond to questions from managers and employees.
Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Ability to make independent decisions when circumstances warrant such action.
Ability to solve practical problems.
Ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
Leadership and supervisory ability.
Willingness to work harmoniously with and supervise other personnel.
Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures necessary for quality care.
Knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.

Ability to relate information concerning a resident’s condition.

Current, unencumbered, active license to practice as an RN in this state.
Current CPR certification.
Must maintain all required continuing education/licensing.
Must remain in good standing with the State Board of Nursing at all times.
Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
Ability to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
May be necessary to assist in the evacuation of residents during emergency situations.

Responsibilities

Supervise the day-to-day nursing activities of the facility.
Ensure nursing activities are in accordance with current federal, state, and local standards, guidelines, and regulations.
Assist the Director of Nursing Services in directing the day-to-day functions of the nursing activities.
Participate in developing, maintaining, and updating written policies and procedures for the nursing service department.
Ensure reference material at nurses’ stations is current and recommend necessary written material.
Ensure the Nursing Service Procedures Manual is current and reflects daily procedures.
Ensure all nursing service personnel comply with the procedures set forth in the Nursing Service Procedures Manual.
Make written and oral reports/recommendations to the Director concerning the operation of the nursing service department.
Periodically review the department’s policies, procedure manuals, job descriptions, etc., and make recommendations for revisions.
Assist in developing methods for coordinating nursing services with other resident services.
Ensure all nursing service personnel are in compliance with their respective job descriptions.
Participate in the development, maintenance, and implementation of the facility’s quality assurance program for the nursing service department.
Participate in facility surveys (inspections) made by authorized government agencies.
Periodically review the resident’s written discharge plan and participate in updating it.
Assist the Director in planning the nursing services portion of the resident’s discharge plan.
Meet with the nursing staff and support personnel to plan department services, programs, and activities.
Admit, transfer, and discharge residents as required.
Complete accident/incident reports as necessary.
Interpret departmental policies and procedures to personnel, residents, visitors, and government agencies.
Perform administrative duties such as completing medical forms, reports, evaluations, studies, and charting.
Agree not to disclose assigned user ID code and password and promptly report suspected or known violations.
Agree not to disclose resident’s protected health information and promptly report suspected or known violations.
Report any known or suspected unauthorized attempt to access facility’s information system.
Attend various committee meetings of the facility (e.g., Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment) as required.
Assist in preparing written and/or oral reports of nursing service programs and activities to submit to committees.
Provide information to the Quality Assurance and Assessment Committee as requested.
Participate in developing the agenda for the Care Plan Committee meetings.
Participate in regularly scheduled reviews of discharge plans and make them available to the Care Plan Committee.
Implement recommendations from established committees as instructed by the Director.
Ensure a sufficient number of licensed practical and/or registered nurses are available for the tour of duty.
Ensure a sufficient number of certified nursing assistants are available for the tour of duty.
Develop work assignments and/or assist in completing and performing such tasks.
Delegate administrative authority, responsibility, and accountability to each Charge Nurse.
Ensure all nursing assistants are enrolled in or have graduated from an approved nursing assistant training program.
Ensure all nurse aide trainees are under the direct supervision of a licensed nurse.
Make daily rounds of the nursing service department to ensure personnel are performing work assignments correctly and report findings to the Director.
Monitor absenteeism to ensure adequate staffing and report problem areas to the Director.
Ensure established policies and procedures, including dress codes, are followed by departmental personnel.
Review complaints and grievances made or filed by department personnel and make appropriate reports.
Develop and maintain good working rapport with inter-department personnel and other departments.
Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment.
Meet with the shift on a regularly scheduled basis to identify and correct problem areas.
Review and evaluate the department’s workforce and make recommendations to the Director.
Participate in employee performance evaluations, determining staffing requirements, and making recommendations concerning dismissals and transfers.
Ensure departmental disciplinary action is administered fairly.
Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
Report known or suspected incidents of fraud.
Ensure departmental computer workstations left unattended are properly logged off or password protected.
Inform nursing service personnel of new admissions, their expected time of arrival, and room assignment.
Ensure rooms are ready for new admissions.
Greet newly admitted residents and escort them to their rooms.
Make rounds with physicians as necessary.
Encourage attending physicians to record and sign progress notes and orders timely.
Inform the Director when physician visits are not made timely.
Consult with the resident, family, and physician in planning resident’s care, treatment, and rehabilitation.
Notify the resident’s attending physician and next-of-kin of changes in the resident’s condition.
Visit residents daily to observe and evaluate their physical and emotional status.
Ensure direct nursing care is provided by qualified personnel.
Review nurses’ notes for informativeness, descriptiveness, and reflection of resident response to care.
Provide direct nursing care as necessary.
Start IVs, obtain specimens for lab tests as ordered.
Take TPRs, blood pressures, etc., as necessary.
Admit, transfer, and discharge residents as necessary.
Report problem areas to the Director and assist in developing and implementing corrective action.
Keep the Director informed of resident status and other related matters.
Assist the Charge Nurse in monitoring seriously ill residents.
Authorize the use of restraints when necessary and in accordance with facility policies.
Ensure RNs and LPNs comply with procedures for administration, storage, and control of medications and supplies.
Monitor medication passes and treatment schedules.
Review medication cards for completeness, accuracy, and adherence to stop order policies.
Inspect nursing service treatment areas daily for cleanliness and safety.
Ensure residents unable to call for help are checked frequently.
Meet with residents and/or family members as necessary and report problems to the Director.
Assist in arranging transportation for discharged residents.
Inform family members of the death of a resident.
Call funeral homes when requested by the family.
Participate in developing, planning, conducting, and scheduling in-service training classes.
Assist the In-service Director/Educator in developing annual facility in-service training programs.
Assist the Director in planning clinical supervision for nurse aide trainees.
Attend and participate in continuing education programs.
Assist the Safety Officer in developing safety standards for the nursing service department.
Assist the Director and/or Infection Control Coordinator in identifying, evaluating, and classifying tasks with potential exposure to blood/body fluids.
Ensure personnel performing tasks with potential exposure participate in appropriate in-service training.
Ensure an adequate supply of personal protective equipment is available.
Ensure personnel follow established procedures for the use and disposal of personal protective equipment.
Monitor personnel to ensure they are following established safety regulations.
Ensure nursing service personnel comply with established departmental policies and procedures.
Ensure nursing service work areas are maintained in a clean and sanitary manner.
Ensure all resident care rooms, treatment areas, etc., are maintained in a clean, safe, and sanitary manner.
Ensure nursing service personnel follow established handwashing and hand hygiene procedures.
Ensure nursing service personnel participate in and conduct fire safety and disaster preparedness drills.
Participate in the development, implementation, and maintenance of the infection control program.
Ensure nursing service personnel follow established infection control procedures when isolation precautions are necessary.
Participate in the development, implementation, and maintenance of procedures for reporting hazardous conditions or equipment.
Ensure personnel wear and/or use safety equipment when lifting or moving residents.
Report missing/illegible labels and MSDSs to the safety officer.
Recommend equipment and supply needs of the department to the Director.
Ensure an adequate stock level of medications, medical supplies, and equipment is maintained.
Assist the Maintenance Director in developing a preventive maintenance program for nursing service equipment and supplies.
Participate in the development and implementation of procedures for the safe operation of all nursing service equipment.
Ensure only trained and authorized personnel operate nursing service equipment.
Ensure all personnel operate nursing service equipment in a safe manner.
Monitor nursing procedures to ensure supplies are used efficiently and waste is avoided.
Ensure appropriate MSDSs are on file for hazardous chemicals.
Participate in the development of written preliminary and comprehensive assessments of resident nursing needs.
Participate in the development of a written plan of care for each resident.
Encourage resident participation in the development and review of their care plan.
Review resident care plans for appropriate goals, problems, approaches, and revisions.
Ensure all personnel involved in resident care are aware of the resident’s care plan and refer to it prior to administering daily care.
Review nurses’ notes to determine if the care plan is being followed.
Assist the Resident Assessment/Care Plan Coordinator in planning, scheduling, and revising the MDS.
Review resident’s medical and nursing treatments to ensure they are provided in accordance with the care plan and wishes.
Keep abreast of economic conditions and recommend adjustments in nursing services.
Report suspected or known incidence of fraud relative to billings, cost reports, kickbacks, etc.
Assist with the overall supervision and management of the nursing staff.
Be a supportive team member, contribute to and be an example of teamwork and team concept.
Maintain a liaison with residents, families, and support departments.
