About The Position

Concierge medicine at Corewell Health offers a holistic and practical approach to health care through personalized health assessments, comprehensive diagnostic and lab testing and health coaching. We limit our practice size to give you the attention, expertise and care you deserve. This team member will provide care management services as well as provide support to the clinical care team in patient communication, direct patient care, scheduling, and referrals.

Requirements

• Associate's Degree Nursing
• LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire
• CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days Required or CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days Required

Nice To Haves

• Bachelor’s Degree Nursing
• Minimum one year of nursing experience
• Care Management background

Responsibilities

• Provide care management services.
• Provide support to the clinical care team in patient communication, direct patient care, scheduling, and referrals.
• Use critical thinking skills and the nursing process to assess, diagnose, plan, implement, and evaluate patient care.
• Uphold the standards of nursing excellence as described by the ANA, including the code of ethics.
