About The Position

Join BrightStar Care of The Woodlands/N Houston as a skilled RN Wound Care Nurse to provide specialized wound, skin, and ostomy care in patients' homes throughout Spring, The Woodlands, and surrounding N. Houston communities. This role is ideal for a nurse who enjoys working independently, building meaningful relationships with patients, and utilizing clinical expertise to improve patient outcomes and quality of life. Requirements

  • Current, unrestricted Texas Registered Nurse (RN) license
  • Current BLS/CPR Certification (in-person certification required)
  • Negative TB skin test or chest X-ray within the past year
  • Minimum one (1) year of documented nursing experience in a home health agency, primary care clinic, wound care clinic, hospital, or healthcare facility preferred
  • Strong wound assessment and treatment skills
  • Knowledge of evidence-based wound care practices and skin integrity management
  • Current and valid Texas Driver's License
  • Current automobile insurance
  • Reliable transportation
  • Strong communication, assessment, and documentation skills

Responsibilities

  • Perform comprehensive wound, skin, ostomy, and continence assessments
  • Evaluate patient skin condition and wound care needs upon admission and on an ongoing basis
  • Develop, implement, and update individualized wound care plans based on clinical findings and physician orders
  • Establish measurable goals and recommend appropriate nursing interventions to promote healing and prevent complications
  • Perform wound care treatments and dressing changes according to physician orders and evidence-based practices
  • Monitor wound healing progress and identify changes requiring intervention
  • Educate patients, family members, and caregivers on wound management, skin integrity, infection prevention, and treatment plans
  • Collaborate with physicians and other healthcare professionals regarding wound care recommendations and patient progress
  • Communicate significant changes in patient condition to the Director of Nursing and other appropriate care team members
  • Maintain accurate, detailed, and timely clinical documentation
  • Adhere to agency policies, infection control standards, and nursing practice regulations
  • Treat patients, families, caregivers, and team members with professionalism, courtesy, and respect
