About The Position
BrightStar Care of Kingwood/Humble is seeking a skilled RN Wound Care Nurse to provide specialized wound, skin, and ostomy care to patients in their homes. This role is ideal for a nurse who enjoys working independently, building meaningful relationships with patients, and utilizing clinical expertise to improve patient outcomes and quality of life. Requirements
- Current, unrestricted Texas Registered Nurse (RN) license
- Current BLS/CPR Certification (in-person certification required)
- Negative TB skin test or chest X-ray within the past year
- Minimum one (1) year of documented nursing experience in a home health agency, primary care clinic, wound care clinic, hospital, or healthcare facility preferred
- Current and valid Texas Driver's License
- Current automobile insurance
- Reliable transportation
- Strong communication, assessment, and documentation skills
Nice To Haves
- Wound, Ostomy, and Continence Nursing Certification (WOCN) preferred
Responsibilities
- Perform comprehensive wound, skin, ostomy, and continence assessments
- Evaluate patient skin condition and wound care needs upon admission and on an ongoing basis
- Develop, implement, and update individualized wound care plans based on clinical findings and physician orders
- Establish measurable goals and recommend appropriate nursing interventions to promote healing and prevent complications
- Perform wound care treatments and dressing changes according to physician orders and evidence-based practices
- Monitor wound healing progress and identify changes requiring intervention
- Educate patients, family members, and caregivers on wound management, skin integrity, infection prevention, and treatment plans
- Collaborate with physicians and other healthcare professionals regarding wound care recommendations and patient progress
- Communicate significant changes in patient condition to the Director of Nursing and other appropriate care team members
- Maintain accurate, detailed, and timely clinical documentation
- Adhere to agency policies, infection control standards, and nursing practice regulations
- Treat patients, families, caregivers, and team members with professionalism, courtesy, and respect
