About The Position

The PRN Registered Nurse will provide patients support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state and will work with the individual patient, their families, and providers of care to develop and implement individual health plans, and provide support to the patient in self-management. Coordinate care and/or follow-up for patients who receive care in outpatient clinics. Coordination of prescription refill requests and patient education coordination in collaboration with physician staff. The RN will develop a specialty in patient care related to the clinic's populations and acts as a resource. The RN will work with the providers and clinical staff to implement innovative approaches and set priorities to meet patient/family needs. Commitment to patient outcomes results in high levels of performance despite workload changes. PRN Nurses will report to an assigned Clinical Services Manager. PRN Applicant’s Work history will be screened to match the position in which they have applied for. PRN Maximum allowable hours in a floating year is 1000.

Requirements

  • BSN, ADN Degree, or Diploma RN.
  • Major: Registered Nurse.
  • RN with a minimum of one (1) year of RN experience.
  • Current BLS certification.
  • Valid Arkansas Registered Nursing License

Nice To Haves

  • ACLS certification or obtain certification within one (1) year of hire date.
  • Knowledge or experience in PCMH desired.
  • Experience in an ambulatory care setting.

Responsibilities

  • Provides patient support and education focused on disease prevention, health maintenance, and self-management of conditions.
  • Performs clinical assessments, identify health needs, and assist in developing individualized care plans in collaboration with providers.
  • Coordinates follow-up care and referrals for patients seen in the outpatient clinic to ensure continuity of care.
  • Coordinates prescription refill requests and educates patients about medication use, side effects, and interactions (as per clinic policy and physician orders).
  • Helps patients and families understand care plans and instructions for care between visits.
  • Works closely with physicians, nurse practitioners, clinic staff, and other health professionals to provide coordinated, patient-centered care.
  • Serves as a clinical resource by developing expertise in the specialty patient population served by the Women’s Center.
  • Communicates patient progress, follow-up needs, and any changes in condition to providers and team members.
  • Supports patient outcome tracking, assessing progress toward health goals and recommending changes to care plans as needed.
  • Implements innovative care approaches and help set priorities that align with patient and family needs.
  • May perform other duties as assigned.
