About The Position

Four Seasons is a recognized leader in hospice and palliative care, and an excellent employer in Western North Carolina. This role provides nursing coverage to patients, families, and community partners via phone and in-person visits, including admission to hospice services, as needed. The position serves as the first responder for symptom-based calls from current patients after normal office hours, providing guidance or communicating the need for follow-up. The RN will participate in the implementation and coordination of patient's hospice care, assist with emergency admissions, and assess the patient's physical and mental health, including their reaction to illnesses and treatment regimens.

Requirements

Must be willing and able to care for patients with infectious disease processes unless approved accommodations
Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
Knowledge of HIPAA privacy rules and regulations
Knowledge of electronic medical records systems
Demonstrate basic data entry skills

Demonstrate understanding of reimbursement, regulatory requirements, and Medicare Conditions of Participation
Two years of clinical experience preferred
Valid NC RN license
Valid Driver’s License and proof of automobile insurance

Responsibilities

Practice within the scope of license, certification or registration, where applicable, and as required by law and regulation
Serve as first responder for symptom-based calls from current patients after normal office hours
Provide guidance to patients and families as needed or communicate need for case manager or designee follow up promptly
Participate in implementation and coordination of patient’s hospice care
Assist with emergency admissions
Assess the patient's physical and mental health including the patient's reaction to illnesses and treatment regimens.
Record and report the results of the nursing assessment.
Plan, initiate, deliver, and evaluate appropriate nursing acts.
Teach, assign, delegate to or supervise other personnel in implementing the treatment regimen.
Collaborate with other health care providers in determining the appropriate patient and family-centered plan.
Implement the treatment and pharmaceutical regimen prescribed by any person authorized by State law to prescribe the regimen.
Provide teaching and counseling about the patient's health.
Report and record the plan for care, nursing care given, and the patient's response to that care.
Supervising, teaching, and evaluating those who perform or are preparing to perform nursing functions and administering nursing programs and nursing services.
Providing for the maintenance of safe and effective nursing care, whether rendered directly or indirectly.
Works to achieve performance goals, pursues progress toward individual development plan (IDP) and demonstrates Four Seasons values and competencies
Participate in department and staff meeting as required
Participate in ongoing education and training offered by the agency and/or required by nursing supervisor as part of individual education plan of RN
Comply with company policies, procedures, guidelines, and Mission Statement
Practice safety and infection control methods
Assist in providing 24 hour on-call services
Assess for the need of other disciplines
Perform other but similar duties as assigned
