About The Position

Tapestry Hospice & Palliative Care is a family-owned and operated organization serving North Georgia for over 16 years. We operate as a Care Family, treating patients with dignity and compassion. We are seeking exceptional individuals to join our growing team, where you will feel appreciated and be part of a dedicated group focused on providing compassion, comfort, and peace to patients and their families. Your role will involve creating, planning, and directing the hospice care team using the nursing process (assessment, interventions, implementation, and evaluation). You will collaborate effectively with the Interdisciplinary Team, patients, and their loved ones, upholding the highest standards of care and professional nursing competency. As an On-Call RN, you will provide holistic nursing care to terminally ill patients and their families outside of normal business hours, offering appropriate support during times of death. Nursing care is delivered through the nursing process, including patient assessments, diagnosis of key problems, patient education, and evaluation of patient outcomes.

Requirements

  • Certified Registered Nurse.
  • Ability to work a Friday - Sunday schedule.
  • Excellent clinical assessment skills.
  • Performs admission evaluations and pronouncements.
  • Ability to provide patient education and evaluate patient outcomes.
  • Time management skills.
  • Advanced knowledge of pain management & symptom control.

Nice To Haves

  • Hospice case management.
  • Critical Care, Surgery or Oncology experience.

Responsibilities

  • Create, plan and direct the hospice care team utilizing nursing processes of assessment, interventions, implementation and evaluation.
  • Effectively interact with all members of the Interdisciplinary Team as well as with your patients and their loved ones while maintaining the highest standards of care and professional nursing clinical competency.
  • Provide holistic nursing care to terminally ill patients and their families after normal business hours.
  • Provide appropriate support for families at time of death.
  • Conducts follow-up visits.
