Requirements
- Candidates must possess at least a bachelor's degree from an accredited program.
- A minimum of three (3) years’ experience in direct patient care in an acute care setting; additional experience in Case Management/Utilization Management/Discharge Planning is required.
- Current licensure as a Registered Nurse in the state of Florida.
Nice To Haves
- A bachelor's degree in a health care related curriculum is preferred.
- BLS is recommended, but not required.
Responsibilities
- Assesses the care needs of patients upon entry into the organization and facilitates appropriate progression through the hospital's continuum of care into the post-hospital environment.
- Collaborates with other members of the health care team in planning, coordinating, and facilitating the implementation of a plan of care based on the reason for admission and current, emerging and projected care needs.
- Performs duties in a manner which enhances service excellence including quality clinical outcomes for patients, efficiencies in hospital operations, and cost-effectiveness.
- Demonstrates accountability for services provided through participation in the monitoring and evaluation of patient care activities.
- Communication and interpersonal skills, basic computer skills, and customer service skills required.
