About The Position

Under the supervision of the Triage Supervisor and the direction of the Triage Nurse, the RN Triage Runner makes hospice care visits after hours to patients in various settings for assessment, symptom management, treatment, teaching and coordination of care. Locations usually include home visits and long term care facilities in region.

Requirements

  • Must currently be registered in the State of Florida as a Registered Nurse.
  • Current State of Florida drivers license with driving record acceptable to Community Hospice insurance carrier, and reliable transportation
  • Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying.
  • Typical qualifications would be equivalent to: Bachelor’s Degree in Nursing or 2 year Associate's Degree; possession of a degree from a diploma nursing program
  • Minimum of two (2) years experience in acute care setting, oncology, home health setting, and hospice preferred.
  • Knowledge of principles and practices of nursing.
  • UF Shands credentialing required after starting employment

Responsibilities

  • Completes assessment of patient during visits for effective symptom management.
  • Perform treatments/interventions in accordance with the Plan of Care and physician’s orders.
  • Provides emotional support to patient/caregiver, teaching patient/caregiver as appropriate to plan of care.
  • Follows Standard Precautions, reporting to Triage Nurse following patient visits.
  • Calls physician as necessary for change in Plan of Care, faciliating changes to continuous care when necessary.
  • Accurately completes documentation of visits/phone communications and submits documentation in a timely manner.
  • Follows policy when patient is transferred to acute care after hours.
  • Arranges for medications and supplies in an emergency
  • Answers phone calls/pages within 15 minutes.
  • Develops basic knowledge of Hospice Medicare/Medicaid and other reimbursement mechanisms.
  • Organizes workload efficiently.
  • Functions effectively in emergency situations and seeks additional responsibilities as appropriate.
  • Uses supplies and equipment appropriately in a cost-effective manner.
  • Displays courtesy, tact and patience, keeping neat and legible written documentation.
  • Attends staff meetings on regular basis.
  • Makes visits at time of death when appropriate
  • Leaves accurate and detailed messages in voice mail boxes for all appropriate team members after making all visits including visits relating to a patient's death.
  • Develop areas of expertise in hospice nursing skills, pursueing professional growth through academic and continuing educational programs.
  • Other duties as assigned
