About The Position

The primary purpose of this role is to be responsible for the delivery of patient care in accordance with current applicable Federal, State, and local standards, guidelines and regulations and established policies and procedures as may be directed by the Administrator, to assure that the highest degree of quality care and safety can be maintained. This role supervises LPNs, CNAs, and Medical Assistants, and reports to the Nurse Manager or Practice Manager.

Requirements

  • Bachelor's Degree Nursing required or Associate's Degree Nursing required
  • Registered Nurse [RN - State License] required
  • Basic Life Support [BLS] required
  • A minimum of one year experience as an RN
  • Must be able to read, write legibly, speak clearly, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action in a timely fashion.
  • Must possess the ability to deal tactfully with staff, patients, family members, visitors, government agencies/personnel and the public.
  • Must possess the willingness to work harmoniously with professional and non-professional personnel and the public.

Responsibilities

  • Deliver patient care in accordance with current standards, guidelines, and regulations.
  • Complete nurse visits as assigned.
  • Document assessment of patient at time of visit, including nursing intervention, patient response, effectiveness, and complications.
  • Communicate patient information to the provider.
  • Implement clinical and technical aspects of care and nursing orders.
  • Intervene with proper technique, procedures, and safety precautions.
  • Implement physician's orders.
  • Manage patient care in the department.
  • Coordinate with providers, Practice managers, PSCs, Department Coordinators, and MAs and delegate tasks as needed.
  • Perform AED/O2 checks in the department.
  • Manage incoming clinical needs faxes for the department, including PAs, DME, and VNA.
  • Separate and distribute provider faxes.
  • Coordinate care with outside agencies with the assigned care team.
  • Consult with in-house resource personnel and departments for treatment.
  • Respect patient dignity and confidentiality, acting as an advocate.
  • Provide a safe and secure environment for the patient.
  • Perform in a calm and professional manner during crisis and/or emergency situations.
  • Function within the limits of the State Nurse Practice Act.
  • Review patient and family knowledge of diseases, illness, or readiness to accept care for the patient at home.
  • Identify needs for care or alternatives such as outpatient treatment or extended care facility.
  • Set realistic learning objectives by including the patient and family in instruction planning.
  • Provide self-care instructions.
  • Identify necessary medical information to be communicated to community resources.
  • Use individualized techniques designed to meet specific patient needs.
  • Initiate referrals to healthcare providers and other resource agencies.
  • Adhere to nursing dress code and wear identification badge.
  • Demonstrate organizational skills and adapt to changing workloads.
  • Participate in the plan of care with the entire team.
  • Administer medications/immunizations.
  • Perform nursing procedures.
  • Provide patient education.
  • Maintain open communication channels with colleagues and physicians.
  • Communicate factual and pertinent information in reports.
  • Identify own learning needs and seek opportunities for self-growth.
  • Use and share skills gained at continuing education classes.
  • Complete initial or annual competencies.
  • Evaluate for age-appropriate behavior, motor skills, and physiological norms of patients.
  • Recognize patient's psychological development and modify approaches for patients.
  • Recognize potential lifestyle changes resulting from illness that will interfere with the care plan and develop a plan.
  • Identify increased physical/emotional dependency needs and evaluate the availability of a support system.
  • Use age-appropriate interview/communication techniques.
  • Assess patients for signs and symptoms of abuse/neglect and refer to appropriate personnel and/or community agency.
