Responsible for the coordination of the care and management of the transplant recipient. Must be able to function with considerable independence in the management of the transplant patient. Responsible for promoting the health and safety of the transplant recipient by ensuring the ongoing communication and collaboration providers and other multidisciplinary team members. Coordinates referrals and follow-up evaluation of each patient; collaborates with the multidisciplinary team to ensure appropriate evaluation per protocol and policies Schedule laboratory tests, X rays, biopsy, etc. according to the established protocols; monitors laboratory results for each patient and responds per protocol or notifies transplant provider of abnormalities and coordinators follows up as directed Promptly notifies patient (and consultants and other treaters) when patient care issue noted Performs phone triage of patient calls and MD calls with ongoing education to patients, shares information regarding protocols and general knowledge with PCP’s or other community treaters Communicates with other team members to ensure continuity of care is maintained Communicates the results of lab tests and biopsies to the patient and their families Communicates on an ongoing basis with patients and documents in Transplant Data Base and appropriate hospital and clinic EMR’s conversations with patient, uses To-Do function Works with multidisciplinary team members in assuring that the psychosocial and financial needs of the patient are being addressed Maintains accurate and current data base on each patient recording all laboratory, diagnostic tests, biopsy results and changes in medications
