This RN Transition Navigator position functions as the liaison and communicator with the patient, caregivers, healthcare providers, multi-disciplinary team members, as well as post-acute care and third-party payers. The role involves discussing alternative care options with patients, caregivers, and the multi-disciplinary team, and assisting with discharge planning needs. The primary goal is to facilitate patient movement along the healthcare continuum to ensure quality, cost-effective outcomes are achieved in collaboration with the multi-disciplinary team. Requirements

  • Graduate of an accredited school of nursing.
  • 2 years of related experience.
  • Current registered nurse (RN) license in state of practice.
  • Basic Life Support (BLS) through the American Heart Association.
  • Proficiency in using computers, software, and web-based applications.
  • Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
  • Good organizational and time management skills.
  • Conflict resolution skills and ability to demonstrate good judgement and decision making in performing tasks.

Nice To Haves

  • Bachelor’s degree in nursing.
  • Certification in Case Management (CCM).
  • Health Care Management (CPHM).

Responsibilities

  • Manages various aspects of the planning process in anticipation of the patient’s movement through the healthcare system.
  • Uses care management policies and procedures including application of evidenced based criteria and level of care and continued appropriateness.
  • Manages the care of assigned patients through the healthcare system based on the patient’s individual needs.
  • Prepares, executes, and reinforces post-discharge care plan.
  • Adapts behavior to the specific patient population, including but not limited to respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.
  • Performs other related duties as required.
  • Remains knowledgeable on current federal, state and local laws, accreditation standards or regulatory agency requirements that apply to the assigned area of responsibility and ensures compliance with all such laws, regulations and standards.
  • Maintains and complies with the company's Compliance & Privacy Program and Standards of Conduct, including the immediate reporting of any known or suspected unethical or questionable behaviors or conduct; patient/employee safety, patient privacy, and/or other compliance-related concerns.
