About The Position

We are seeking a compassionate and skilled Registered Nurse (RN) Team Nurse to join our hospice care team. This full-time position works Monday through Friday from 8:00 a.m. to 5:00 p.m. and includes rotating weekend and holiday coverage. The RN Team Nurse provides clinical support to RN Care Managers, assists with scheduled patient visits, and helps ensure continuity of care when primary care managers are unavailable. The RN Team Nurse is responsible for delivering high-quality patient care through comprehensive assessments, care planning, implementation of nursing interventions, and ongoing evaluation of outcomes. This role collaborates closely with patients, families, physicians, and interdisciplinary team members to provide compassionate hospice care while adhering to professional nursing standards.

Requirements

  • Graduate of an accredited nursing program
  • Associate Degree in Nursing (ADN/ASN) required
  • Current, unrestricted Registered Nurse license in the State of Ohio
  • Current CPR certification
  • Proficiency with electronic documentation and computer systems
  • Valid driver's license and reliable transportation
  • Safe driving record
  • Proof of state-required automobile insurance coverage
  • Ability to travel in varying weather and road conditions

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred
  • Minimum of two years of acute care nursing experience preferred
  • Hospice and Palliative Nurse Certification (CHPN) preferred or willingness to obtain certification

Responsibilities

  • Conduct comprehensive assessments of patients and families, addressing physical, emotional, psychosocial, and spiritual needs.
  • Create, implement, and regularly update individualized care plans in partnership with patients and caregivers.
  • Monitor patient status and revise care interventions, medications, and visit schedules as clinical needs change.
  • Communicate significant patient condition changes to physicians and assist in determining appropriate levels of care and service eligibility.
  • Participate actively in interdisciplinary team meetings, care conferences, admissions, and care coordination activities.
  • Accurately document physician orders and patient information within the electronic health record system.
  • Provide guidance, oversight, and education to personal care specialists, volunteers, and family caregivers to support adherence to care plans.
  • Complete timely and thorough clinical documentation at the point of care to support continuity and quality of services.
  • Facilitate effective handoff communication using established reporting methods, including SBAR and other clinical communication tools.
  • Assist with departmental and organizational needs as requested to ensure seamless patient care delivery.
  • Support quality improvement initiatives and organizational performance goals.
  • Serve as a preceptor and mentor for newly hired clinical staff when assigned.
