About The Position

Focused Post Acute Care Partners (FPACP) is seeking a dedicated RN Supervisor to join their team at Focused Care at Orange. This role involves assisting in the supervision of nursing services and setting high standards of care to meet the medical, physical, emotional, and psychological needs of each resident. FPACP emphasizes a culture of excellence and recognition through its ROCKStar program, offering opportunities for team members to be recognized and rewarded.

Requirements

A registered nurse licensed and in good standing with the Texas State Board of Nurse Examiners in the state of Texas.

Responsibilities

Supports the Director of Nursing in monitoring, evaluating, and organizing direct nursing care.
Effectively communicates with the Director of Nursing, Medical Doctors, team members, and residents to promote optimal clinical outcomes and continuity of care.
Works cooperatively as part of the interdisciplinary team to assess, develop, implement, and evaluate resident care plans.
Makes frequent rounds throughout the shift and assists residents and staff as needed.
Utilizes critical thinking for resident conditions and implements interventions to prevent avoidable rehospitalizations.
Monitors the completion of electronic medical records/documentation.
Educates staff on policies and processes as necessary.
Supports newly hired nursing staff.
