About The Position

The RN Supervisor Hospice assists the Director in planning, directing, supervising, and monitoring department activities to achieve optimum patient care by meeting established goals and objectives. This role coordinates activities with the Medical Staff, Administration, families, ancillary services, and community agencies to enhance total patient care. WVUHS Home Care provides comprehensive support and peace of mind to individuals and families transitioning from hospital to home, offering Home Health, Hospice, and Home Medical Equipment (HME) services. Requirements

Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
Valid driver’s license.
CPR certification within 30 days of hire date.
Three (3) years of progressive Registered Nurse experience.
Two (2) years Hospice experience.

Nice To Haves

Bachelors of Science in Nursing (BSN).
Two (2) years of previous experience in a hospital/home care/hospice leadership role preferred (such as charge nurse, or documentable consistent demonstration of strong leadership skills while functioning as a staff nurse).

Responsibilities

Maintain working relationships with patients, families, staff, physicians, and referral sources.
Take phone calls and make appropriate documentation of these calls.
Obtain orders from physicians as needed.
Assists with critical lab reporting/monitoring.
Communicates patient care problems or potential problems to the Director occur to assure continuity and coordination of care.
Assumes on-call responsibility per policy/procedure.
Performs essential functions of the RN.
Documents patient status and care or services furnished in appropriate areas, as defined by the documentation policy and procedure.
Monitor supervisory reports for LPH/HHA Compliance.
Completes and documents full system assessment, including physical and psychosocial aspects and identifies age-specific criteria upon admission and each visit thereafter. Monitors filed staff patient care practices.
Performs CPR when directed by patients/appropriate representative.
Demonstrates the ability to order and maintain equipment according to the Policy and Procedure Manual.
Demonstrates bag technique per policy and procedure.
Performs and monitors patient teaching according to the policy and procedure.
Coordinates care so that medications are administered as ordered by a physician and in compliance with the Medication Administration Policy and Procedure.
Consults with Director on difficult cases.
Assists in orienting new staff to the role as needed.
Exercises independent professional judgment as evidences by identifying potential problems and taking appropriate action.
Attends all meetings (as available) including conducting IDT Weekly meetings with accountability for compliance.
Monitors faxed labs. Contacts physician offices of abnormal labs.
Supervises staff schedule as requested, resolving any problems and keeping orders current.
Assists with duties of the director when out of the office including appropriate work queues and reports.
Maintains schedules to assure low to adequate coverage, clear patterns, and adequate notice to Associates to achieve outcomes of department, and oversees productivity.
Upload HIS admission/discharge after Director audit.
Delegates proper task to appropriate person with accountability.
Maintains working knowledge of flow-charting, EPIC, Kronos, Word, and Windows, per department needs.
Assists director with Kronos compliance. Maintains on call and holiday schedule.
