About The Position

This role is for a Hospice House Staff Nurse, specifically for the night shift. The nurse will be responsible for observing, assessing, documenting, and reporting resident conditions and changes. They will implement individualized Plans of Care, identify significant changes in resident conditions and take appropriate action, and oversee Certified Nurse Aides. The role also involves discharging residents, completing nursing assessments before contacting physicians, communicating with resident points of contact, providing staff training, utilizing customer care skills, timely documentation, and ensuring compliance with all relevant laws and regulations.

Requirements

Current State Registered Nurse License
Current Driver's License
Reliable transportation
Provide ongoing valid and current auto liability insurance
Two years of experience as an RN preferred in a healthcare/clinical setting

Responsibilities

During rounds observes, assess, documents and reports resident conditions and/or changes
Implement the individualized Plan Of Care as directed
Identify significant changes in the condition of residents and takes necessary action per facility policies and procedures
Oversees and monitors Certified Nurse Aides
Discharge residents per facility
Completion of Nursing Assessment for Physician Calls prior to calling Physician on any change in resident condition
Communicate with resident’s Point of Contact when they ask for status updates or if there is a change in condition
Provide training to other staff as needed
Utilize excellent customer care skills with internal and external customers, including using proper phone etiquette
Timely submission of all documentation
Performance of other duties as required
Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
