About The Position

Heart Med is a leader in remote patient monitoring, combining advanced technology with personalized care. We offer comprehensive solutions for monitoring critical patient vitals such as blood pressure, pulse rate, and oxygen saturation. We are seeking a detail-oriented Registered Nurse (RN) to support our Remote Patient Monitoring (RPM) program from our office. The RN will review patient-generated health data, identify concerning trends, follow clinical protocols, contact patient care teams, document interventions, and escalate concerns to the supervising provider. The ideal candidate is comfortable with electronic health records and technology-enabled care platforms.

Requirements

  • Current, unrestricted Registered Nurse license in Illinois or a multistate nursing license recognized in Illinois
  • Graduation from an accredited nursing program.
  • Experience using electronic health records and computer-based clinical systems.
  • Strong clinical judgment, organization, and attention to detail.
  • Excellent verbal and written communication skills.
  • Ability to work independently while collaborating effectively with the care team.
  • Ability to follow protocols consistently and recognize when escalation is necessary.
  • Professional telephone manner

Responsibilities

  • Review and interpret remote patient monitoring data in accordance with established clinical protocols.
  • Monitor patient readings, trends, alerts, and compliance with prescribed monitoring schedules.
  • Identify abnormal, missing, or potentially inaccurate readings.
  • Triage patient concerns within the RN’s scope of practice and escalate urgent or clinically significant findings according to protocol.
  • Notify the appropriate provider, physician, advanced practice provider, or care team when readings or symptoms require further evaluation.
  • Respond promptly to high-priority alerts and document all actions taken.
  • Maintain accurate, timely, and complete documentation in the electronic health record and RPM platform.
  • Coordinate with providers, clinical staff, admissions personnel, and other departments as needed.
  • Report recurring technical, clinical, or workflow issues to the appropriate supervisor.
  • Protect patient privacy and maintain compliance with HIPAA and organizational confidentiality policies.
  • Perform other related nursing and administrative duties as assigned.
