About The Position

The staff Registered Nurse (RN) is a professional caregiver who assumes responsibility for a group of patients for a designated time frame and provides care to these patients via the utilization of the nursing process, nursing trends, technology, financial and human resources within the philosophy of the institution. The RN will complete initial assessment upon patient admission to the Rehab unit and develop an appropriate Rehab plan of care in accordance with unit standards and based on patient and family needs. They will implement the Rehab plan of care, nursing interventions, and patient care procedures, and modify the plan of care based on continuous evaluation. This role coordinates nursing care activities in collaboration with other members of the interdisciplinary rehabilitation team to facilitate achievement of overall patient goals. The RN will implement procedures and theories related to Rehab practice and scope of service, maintaining up-to-date and accurate documentation of nursing care provided to assure the integration of information for use by the healthcare team. They will share information about the disease processes underlying disabilities and teach nursing techniques to help the rehab patient and their families develop self-care skills necessary to move toward wellness. Additionally, the RN will participate in Rehab rounds and team meetings, reinforce teaching done by therapy in rehab, provide resource materials for patient's changing needs, and provide on-going patient education. The position requires specialized knowledge and clinical skills necessary to provide care for people with physical disability and chronic illness, and to fulfill all requirements of Rehab departmental competencies.

Requirements

Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
Basic Life Support (BLS) within 30 days - American Heart Association (AHA)
H.S. Diploma/GED
Associate's Degree

Responsibilities

Performs all responsibilities and duties required by unit as specific to the scope of service.
Implements procedures and theories related to the specific area of practice.
Maintains up-to-date and accurate documentation of nursing care provided to assure the integration of information for use by the healthcare team.
Completes initial assessment upon patient admission and develops appropriate plan of care in accordance with unit standards and that is based on patient and family needs.

Takes responsibility for patient and employee safety by reporting and/or correcting safety hazards and incidents.

Identifies own learning needs and takes initiative to begin appropriate solution.
Fulfills all requirements of departmental competencies.
