About The Position
As a Home Health Case Manager, the Registered Nurse delivers hands-on patient care in patients’ homes. The role coordinates services from other healthcare providers, communicates with health insurance companies, and oversees visiting nurses. The nurse uses physical assessment, diagnostic interpretation, and effective communication skills to develop personalized care plans aligned with physician orders and thorough initial assessments. The position collaborates with nurses and professionals in Physical Therapy, Speech Therapy, and Occupational Therapy. The employer says nurses complete charting during visits with support from Roger’s AI-powered system to support work-life balance. The position is available full-time and PRN in Lake Ozark, Missouri, and surrounding areas.
Requirements
- Missouri or Compact Registered Nursing licensure.
- Driver’s license.
- Vehicle with current auto insurance.

Responsibilities
- Manage a caseload, including supervising LPN visits.
- Provide routine visits, evaluations, and starts of care as part of a varied weekly caseload.
- Educate patients about health conditions and care topics, including CHF, COPD, diabetes, wound care, and medication management.
- Communicate with physicians to obtain orders and write diagnosis-driven plans of care.
- Coordinate services from other healthcare providers and communicate with health insurance companies.
- Oversee the work of visiting nurses.
- Collaborate with nurses and the Physical Therapy, Occupational Therapy, and Speech Therapy teams.
- Complete charting during visits using Roger’s AI-powered support.
