Position Summary The RN Staff Nurse is responsible for directing, planning, evaluation and coordination of care for an assigned group of Members. Support Culture of Excellence through active role with assigned neighborhood initiatives.

Requirements

  • Education: Graduation from an approved school of nursing.
  • Experience: At least one year of nursing experience.
  • Skills: Must be capable of lifting
  • Licenses / Certifications: NYS Registration and Licensure.
  • Dependable, report to work on-time for every schedule shift (ready to work at beginning - start of shift) and work full schedule shifts unless physically unable or pre-approved Paid Time Off (PTO) which includes personal and vacation.
  • Willing and able to work as required by the position and/or approved by management.
  • Have reliable transportation to and from work.
  • Must be clean and neat in appearance and have a “friendly” service-oriented demeanor and supports Culture of Excellence initiatives by interactions with co-workers and/or Members.
  • Abide by work-related / job procedures and organization policies; (refer to the Employee Handbook).

Responsibilities

  • Responsible for administration of medications and treatments.
  • Demonstrates excellent communication and interpersonal skills.
  • Understanding of and commitment to continuous quality improvement.
  • Demonstrates knowledge and practice of Nursing Department policies and procedures.
  • Provides first aide at Linden Knoll as needed.
  • Demonstrates excellent assessment skills and the ability to develop a plan of care of a Member/ Patient.
  • Evaluates Member response to care plan, prioritize and implement the appropriate changes to care.
  • Provides on unit teaching to nursing staff as needed.
  • Has oversight, to ensure the completion of Certified Nursing Assistant responsibilities.
  • Demonstrates excellent customer service skills.
  • Ensures that all Members are receiving the necessary care and services to enhance and promote quality of life.
  • Documents all required Member care information accurately and timely in My Unity.
  • Directs and supervises the work of the Nurses and Certified Nursing assistants for assigned shift.
  • Accountable for the quality of care on a specific nursing unit.
  • Assists in the evaluation of Member progress towards expected goals and documents in Medical Record.
  • Provides assistance to medical with Member assessments.
  • Provides information to on call doctor, regarding medical problems and assures that the medical plan of care is carried out.
  • Notifies Member families with changes to Members plan of care.
  • Implements Members plan of care in an accurate, safe and appropriate manner.
  • Participates in the development, implementation, and evaluation of the nursing care plan for Member within the unit.
  • Collaborates with appropriate members of the interdisciplinary team to establish a total plan of care for each Member.
  • Participates in evaluation of work performance of LPN’s and Certified Nursing Assistants on a specific unit.
  • Rearranges unit staff Member assignments as necessary to ensure quality care to all Members.
  • May assist with Nursing Quality audits.
  • Promotes and models the Friendly Home values of integrity, customer focus, excellence, compassion, friendship, and collaboration/team work.
  • Participates in In-service Education and Safety Programs.
  • Updates the Nursing Supervisor of any Member changes as per Nursing Department standard.
  • Complies with HIPPA regulations.
  • Performs other duties as required by Supervisor or by Administration from time-to-time.
  • Accountable to ensure all required daily documentation is completed.
