About The Position

The Patient Flow team at Duke University Hospital is looking to grow! This exceptional group of Clinical Professionals is dedicated to the facilitation and coordination of patient movement within a world-class healthcare system, surrounding regional healthcare systems, and beyond. The Patient Flow team works with referring agencies, hospitals, clinics, and emergency departments throughout the state and beyond to coordinate safe and efficient integration into the Duke University Health System. This team also facilitates patient movement throughout the facility by coordinating transfers between the Inpatient Units, PACU, Emergency Department, and Ambulatory Clinics. This team works alongside providers to identify patient needs, triage needs based on acuity, and ensure that patients are relocated in accordance with their needs. With thousands of requests handled per year, the Duke Health Transfer Center is the patient and provider link to world-renown patient and family centered care. With strengths of teamwork, inter-disciplinary communication and collaboration, the Transfer Center offers a dynamic environment for ongoing development of the experienced Registered Nurse. Our dedicated team provides 24/7 coverage to support the transfer needs of patients while networking with our internal and external customers.

Requirements

BSN degree from an accredited nursing program.
Continuous BLS certification.
At least three (3) years of direct experience working in an inpatient medical/surgical facility.
Three years of Critical Care or ED nursing experience required at a Level 1 Trauma Center.
Current registration with North Carolina State Board of Nursing as a registered professional nurse OR current compact RN licensure to practice in the state of North Carolina.
BLS (or higher) certification required.

Nice To Haves

Experience in the critical or emergency care setting.
Understanding of how to navigate complex medical systems with a focus on patient centered care.
Direct experience working in an Emergency Department, Critical Care Environment, or Transfer Center at a Level 1 Trauma Center.
Knowledge of Duke Health System hospitals and patient care units.

Responsibilities

Facilitate and coordinate patient movement within Duke University Health System.
Coordinate with referring agencies, hospitals, clinics, and emergency departments.
Coordinate transfers between Inpatient Units, PACU, Emergency Department, and Ambulatory Clinics.
Work alongside providers to identify patient needs and triage based on acuity.
Ensure patients are relocated in accordance with their needs.
Provide 24/7 coverage to support patient transfer needs.
Network with internal and external customers.
