DEPARTMENT DESCRIPTION: The Heart Institute at UW Medical Center-Montlake, located in Seattle, WA, offers a specialized Advanced Heart Failure & Transplant Clinic. This program focuses on treating patients with weak hearts and end-stage heart failure using comprehensive care, including medical management, mechanical circulatory support (VADs), and heart transplantation. WORK SCHEDULE: 100% FTE M - F, no weekends or holiday shifts allowing for work life balance in the beautiful northwest community POSITION HIGHLIGHTS: Conveniently located near public transportation Make a difference for the patients and employees we serve Opportunity to provide career growth PRIMARY JOB RESPONSIBILITIES: Provides expert nursing care to heart failure patients Systematically assesses the need for and the delivery of nursing services that meet patient care requirements, whether delivered directly or by the other nursing staff Ability to formulate, coordinate and revise an individualized plan of care as indicated by the patient’s response to treatment and evaluate overall plan daily for effectiveness Provides care for individual patients over the telephone using the nursing process Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors Integrates patient and family education into the delivery of care, ensuring educational activities are culturally appropriate and are tailored to the patient’s and family’s ability to comprehend and participate in care Coordinates the care given by others to achieve efficient clinic operations Assumes responsibility for continued professional growth and sharing expertise with other team members REQUIREMENTS: Active WA State or Multi-state RN License by start date Current BLS and ACLS 2 years of outpatient or inpatient Cardiology/Tele experience ABOUT UW MEDICAL CENTER-MONTLAKE: UW Medical Center is an acute care academic medical center located in Seattle with two campuses: Montlake and Northwest.
