About The Position

This role provides clinical nursing support for the pulmonologist and pulmonology patient population. The nurse navigator serves as the point person for a program dedicated to prevention and protecting those most at risk for lung disease. They act as a patient advocate, offering emotional support and encouragement as patients navigate the healthcare system. Responsibilities include checking in with patients at home to verify appointments and medication adherence, attending appointments as needed to ensure patient understanding of test results, disease processes, and treatment options, and responding to patient and family inquiries. The role also involves arranging referrals for supplementary or community services, working closely with the healthcare team for coordinated patient care, educating patients and families during bronchial thermoplasty treatment, and serving as an educator in the smoking cessation program (requiring certification within one year of hire). Additionally, the role involves evaluating and implementing best practices for survivorship and psychosocial care, and on an as-needed basis, enrolling and following patients in clinical trials.

Requirements

  • Graduate of an accredited nursing program.
  • BSN or upon hire BSN completion agreement for enrollment in an accredited BSN Completion Program
  • If hired prior to 1-1-14 in Winnebago County and prior to 9-1-14 in Rock, Walworth and McHenry Counties an ADN only is required.
  • Minimum 1 year RN experience, preferably pulmonology or critical care.
  • Licensed as an RN in state of practice
  • Approved BLS certification required within 90 days (Other certification per unit requirements: ACLS, PALS, PEARS, NRP)
  • Has strong organizational skills and has the ability to produce effective verbal and written communication with proper grammar/spelling.
  • Ability to read and comprehend written instructions as well as follow verbal instructions.
  • Ability to perform clerical skills such as filing, phones and data entry.
  • Ability to manage stress appropriately and make decisions under pressure.
  • Manages anger/fear/hostility/violence of others appropriately.
  • Handles multiple priorities and works well independently.
  • Basic knowledge of Microsoft Office Processing software.
  • May include color blind test (per department guidelines)

Responsibilities

  • Responds to all inquiries, requests and issues which may arise with patients and families.
  • Arranges referrals for supplementary or community services which may be helpful during treatment.
  • Works closely with all members of the healthcare team to facilitate a coordinated approach for patients' plan of care.
  • Educates patients & families while facilitating the process for patients undergoing bronchial thermoplasty treatment.
  • Serves as active educator in the smoking cessation program, receiving appropriate certification within one year of hire.
  • Evaluates and implements best practices regarding survivorship and psychosocial care.
  • On an as needed basis enrolls and follows patients involved in clinical trials.
